Video Assisted Thoracoscopic removal of a mediastinal mass using LigaSure Vessel Sealers

Objective

A 42 year old female, with no significant past medical history, presented with a 5 month history of unexplained shortness of breath. Chest radiograph demonstrated an enlarged cardiac silhouette, though subsequent echocardiogram returned normal. Thoracic computed tomography suggested a right paracardiac lipoma abutting the right pericardium, totalling 8cm at maximum diameter. A differential diagnosis of an indolent liposarcoma was made. Surgical intervention was agreed upon. Following informed consent, patient underwent a two-port video assisted thoracoscopic removal of the mediastinal mass. Uniquely, a LigaSure Vessel Sealer (Covidien, CO, USA) was utilised to aid soft tissue dissection and vessel ligation. This allowed for improved visualisation of key anatomical landmarks, in addition to prompt haemostasis and rapid surgical progress. LigaClips (Ethicon, NJ, USA) were used to securely isolate and clip thymic vessels. Post operatively, patient progressed with no concerns.
