A case of off-pump CABG with limited conduits, calcified aorta and LMS disease

Objective
Introduction:
Choice of conduit for coronary bypass surgery can be challenging in elderly patients with peripheral vascular disease and impaired mobility, especially when multi-vessel revascularization is needed.
Methods:
We describe the case of an 83 year-old ex professional footballer with peripheral vascular disease and multiple leg traumas mobilising with a Zimmer frame. He underwent uneventful CABGx4 with the use of LIMA, left radial artery and antecubital vein of the arm.
Conclusion:
We discuss the conduit options and suitability and our surgical technique of conduit harvesting and off pump surgery.
